Employer Token Application Form

Employer Access Tokens are alphanumeric tokens that an employer can issue to their employees. Participants on MHPOD who add the token to their profile will have their topic completions included in a monthly report that is emailed to the employer on the first of every month.

IMPORTANT INFORMATION BEFORE SUBMITTING APPLICATION

Before you proceed please make sure you have read the above help page and you have created a shared email address for the purpose of receiving reports. It is a policy of the MHPOD Program that we will not send reports to private/individual email addresses, but only to shared email addresses within your organisation such as learning@yourorganisation or training@yourorganisation.

You will need to enter this email address into the "Report Receiving Email Address" field in order for a token to be generated and assigned to your organisation. You must also ensure that an account using this email address has been created on the MHPOD Learning Portal in order for us to assign the token to it.

I am authorised on behalf of the organisation to make this application for a MHPOD Learning Portal Employer Access Token. I understand that my staff accessing and engaging with the MHPOD Learning Portal do so under its End User License Agreement and that the process of sharing their information will only occur once they update their profile information with my organisation’s Token and provide consent. I understand that my staff and the MHPOD Program can revoke an organisation’s access to this information at any time.
